What You Will Do - Essential Functions

The Provider will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey. This position offers the opportunity to make a significant impact on patient outcomes and promote continuity of care. Visits will be completed in-home and/or community base services such as assisted living facilities, skilled nursing facilities or group homes or other delegated locations.

Please note the position requires you to meet patients in their homes in Pinellas County, FL.

  • Clinical Care & Patient Management
    • Conduct comprehensive in-home visits (minimum 25/week) including medical history, physical exams, and medication reviews in assigned territory.
    • Perform thorough patient assessments, formulate diagnoses, and develop individualized treatment plans.
    • Manage chronic conditions (e.g., diabetes, hypertension), including ongoing monitoring and education.
    • Prescribe and adjust medications, educate on usage and side effects, and ensure preventive care measures.
    • Serve as the primary care provider of record and conduct follow-up appointments to evaluate treatment outcomes.
  • Patient Engagement & Collaborative Care
    • Provide patient education on disease management, healthy living, and self-care strategies.
    • Exhibit a compassionate bedside manner to build trust with patients and families.
    • Collaborate with physicians, nurses, and healthcare teams to ensure coordinated, high-quality care.
    • Participate in after-hours on-call duties and maintain flexible availability for longitudinal and urgent care visits.
  • Compliance, Documentation & Operational Responsibilities
    • Maintain accurate, timely EMR documentation (within 48 hours of visits).
    • Ensure reliable transportation for home visits and adherence to scheduled appointments and assigned territories.
    • Follow all company policies, healthcare regulations, privacy/confidentiality laws, and ethical guidelines.
    • Engage in quality assurance activities and perform other duties as assigned to support business needs.
Who You Are

  • Required Qualifications
    • Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.
    • Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant,
    • Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements. Preferred two years of experience in a primary care or longitudinal care setting.
    • Active Drug Enforcement Administration (DEA) license.
    • Current Basic Life Support Certification for health care providers.
    • Strong clinical assessment and diagnostic skills.
    • Excellent communication and patient education abilities.
    • Proficiency in electronic medical record (EMR) systems.
    • Valid proof of sufficient car insurance coverage for the state that appointments are scheduled to be completed in with access to reliable transportation.
    • Valid and active driver’s license for a minimum of 3 years without restrictions, and minimum of 7 years with no DUIs or other felony driving convictions.
    • Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

    • Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
    • Ability to document assessment findings accurately and thoroughly.
    • Ability to communicate effectively through verbal and written communication to members and the clinical team.
    • Demonstrate empathy and respect for members while completing in-home assessments
    • Maintain member, team member and employer confidentiality while remaining in compliance with all HIPAA regulations.
    • Ability to make critical decisions while following Emcara Health policies and procedures.
    • Ability to adapt easily to changing conditions and work responsibilities.
    • An ability to display a positive attitude and maintain a collaborative relationship with health plan members and the clinical team.
    • Ability to proactively anticipate and address/resolve member, provider, and health plan issues
    • Demonstrated clinical skills necessary to perform in-home assessment.
  • Working Conditions
    • Ability to lift and carry up to 20 lb
    • Ability to sit, stand, bend, and/or kneel to perform assessments.
    • Additional demands include constant ability to remain in a stationary position, moving about inside and outside of member homes, navigating stairs and a variety of dwelling conditions, ability to communicate and exchange information with others, ability to inspect information, ability to perform repetitive motion with arms and fingers, and drive to and from member homes.
    • Mental demands include constant ability to interpret data, problem solve, make decisions, and organize and plan.
    • Work is performed in member homes and potentially exposed to adverse environmental conditions.
