As a Case Manager RN, you will facilitate care coordination, advocate for patients, and help optimize healthcare outcomes. This role is ideal for compassionate professionals who excel in communication, collaboration, and problem-solving.
What You'll Do
Assess and prioritize patient care needs, ensuring timely interventions.
Provide virtual care coordination and support to patients, families, and caregivers.
Identify and assist with the follow-up of high-risk patients across various healthcare settings.
Collaborate with physicians and interdisciplinary teams to develop and implement care plans.
Utilize disease management protocols and best practices to improve patient outcomes.
Coach patients using motivational interviewing techniques to enhance self-care and adherence to treatment plans.
Coordinate discharge planning, including home health services, follow-up appointments, and social services.
Maintain clear and effective communication with healthcare providers, patients, and families.
Document all patient interactions and care plans in electronic health records.
Actively participate in Care Management Coordination Committee meetings.
What We're Looking For
Required:
Active RN license in current state of residence (compact licensure preferred).
At least 2 years of nursing experience in a hospital, acute care, or direct care setting.
Experience with renal care, chronic kidney disease, or dialysis patients.
Proficiency in Microsoft Office (Word, Excel, Outlook) and the ability to multitask in a virtual environment.
A dedicated workspace with reliable high-speed internet.
Availability to work evening shifts as needed.
Preferred:
Bachelor's Degree in Nursing (BSN).
Certified Case Manager (CCM) certification.
Experience in ICU, cardiology, or critical care.
Telephonic case management or discharge planning experience.
Knowledge of hypertension and diabetes management.
Diabetic educator experience.
