Responsibilities & Duties:

  • Providing intensive care management to foster care youths of all ages
  • Communication and relationship building with youths, foster parents, and collateral partners
  • Development if interventions and goal setting
  • Participate in monthly team meetings
  • Document encounters daily in conjunction with quarterly paperwork and care meetings
  • Provide after-hours on-call support on a regular, rotating basis

Required Skills & Qualifications:

  • Bachelor's Degree in the human service or behavioral science field
  • LSW, LSW-A, or license-eligible
  • Familiarity with trauma-informed care
  • Valid driver's license and ability to clear DMV, DCF, and CORI background checks

Led by a passionate and involved leadership team based out of our Easthampton location.
