Position Overview:
As a Licensed Mental Health Counselor, you will be an integral part of a collaborative care team, reporting to the Behavioral Health Leadership. Your role will involve working closely with patients and primary care providers to develop, implement, and manage personalized care plans that align with each patient's unique needs and goals. You will also partner with health advocates to perform biopsychosocial assessments and assist in overcoming barriers to accessing medical and behavioral health care.
In this position, you'll leverage your expertise in behavioral health to connect patients with community resources, guide families, and act as a liaison between the patient and external healthcare providers. Your work will be crucial in supporting patients and their families in navigating the healthcare system and ensuring adherence to their care plans.
Key Responsibilities:
Conduct in-person and virtual consultations with patients, understanding their personal and social needs, and connecting them with relevant resources and support.
Collaborate with an interdisciplinary care team to develop care plans tailored to individual patient needs and goals.
Offer guidance and support to your team, ensuring effective communication and care coordination.
Perform psychosocial assessments, including mental status exams, and translate clinical observations into actionable care plan goals.
Provide crisis intervention and short-term evidence-based therapy when necessary.
Build and maintain strong relationships with community organizations to ensure patients receive the support they need.
Travel to patient visits across Syracuse, NY, and surrounding counties, with approximately 60% of time spent in the field. The remaining time will be spent working remotely.
Travel Requirement:
This role involves field-based patient care, requiring personal transportation or the use of a mobile unit for patient visits in the Syracuse area.
Compensation:
$60,000 - $70,000 annually

Preferred Qualifications:
Active license to practice as a Licensed Mental Health Counselor (LMHC)
1 to 3 years of post-master's experience in behavioral health, particularly in community-based or field-based care settings
Experience working with individuals with serious mental illness (SMI) and substance use disorders (SUD)
Proficient in conducting psychosocial assessments and creating tailored care plans
