About the Role
We're looking for a compassionate and experienced Licensed Mental Health Counselor to join our collaborative care team. In this role, you'll work directly with patients to craft, implement, and maintain personalized care plans that address their unique needs and goals. You will be an integral part of a multidisciplinary team, working alongside primary care providers and health advocates to ensure patients have access to both medical and behavioral health care.

Your expertise in behavioral health will be key in building strong relationships with external organizations to ensure patients adhere to their care plans. You will also play a vital role in educating patients and their families on navigating the healthcare system and connecting them to necessary resources.

Key Responsibilities

Conduct patient consultations, both virtually and in-person, including home visits, to understand their personal circumstances and social needs, matching them with appropriate resources
Collaborate with the care team to develop and refine personalized care plans that align with each patient's goals
Provide support and guidance to an interdisciplinary care team, fostering an environment of teamwork and shared purpose
Conduct psychosocial evaluations, mental status exams, and crisis interventions as needed, providing short-term, evidence-based therapy
Establish and nurture partnerships with local community organizations to enhance patient care
Perform 60% of work in the field visiting patients in Syracuse, NY, and surrounding counties, with the remaining 40% spent working remotely
Utilize personal transportation or a team mobile unit to travel to patient visits
Compensation and Benefits

Salary: $60,000 - $70,000

To be successful in this role, you should have:

An active license to practice as a Licensed Mental Health Counselor
1-3 years of post-master's experience working with behavioral health populations, preferably in a behavioral health or integrated primary care setting
Experience working in community-based or field-based care
Ability to assess and intervene with individuals experiencing Serious Mental Illness (SMI) and Substance Use Disorders (SUDs)
Experience conducting psychosocial assessments and developing care plans
