As a Medical Social Worker at Home Health Care Solutions, you are responsible for the assessment of social and emotional factors related to the patient's illness, need for adjustment to care and response to treatment. Assessment of the relationship of the patient's nursing and other medical requirements to the patient's individual home environment, psychological/cognitive condition, financial resources and available community resources.

- Graduate of an accredited school of social work with a Master's degree
- Current licensure as a Certified Social Worker
- One year social work experience (post master's) in health care or related sitting
