Shifts:

1. Days

Key Responsibilities:

1. Provide comprehensive wound care to residents, including assessment, planning, treatment, and evaluation.
2. Collaborate with the interdisciplinary team to develop and implement wound care plans.
3. Monitor wound healing progress and make adjustments to care plans as needed.
4. Educate residents and their families on wound care and prevention strategies.
5. Maintain accurate and timely documentation in compliance with facility policies and healthcare regulations.
6. Serve as a resource for staff regarding wound care best practices and protocols.
7. Ensure compliance with infection control procedures and maintain a clean environment.

Qualifications:

1. Current LPN license in good standing.
2. Wound Care Certification preferred or willingness to obtain.
3. Proven experience in wound care, preferably in a long-term care or rehabilitation setting.
4. Strong clinical skills with a focus on patient-centered care.
5. Excellent communication and organizational abilities.
6. Commitment to professional growth and staying updated on the latest wound care practices.
